My name is Thomas Barhorst, and I started counseling in 2004. In addition to having a small private practice shortly after I received my Missouri license in 2007; I have worked as a K-12 school counselor, in substance use treatment, in a residential treatment setting, in a community mental health agency, helping individuals with disabilities transition into the workforce, and helping graduate students manage stress in their professional programs (i.e. medical school, dental school, etc.). Recently, I have begun providing telehealth therapy after obtaining licensure in Arizona and California. Outside of work, spending time with family and friends is important to me. I also enjoy stories, laughing, listening to stand-up comedy, cooking, playing sports like wallyball and ultimate frisbee on a non-competitive level, watching film noir and psychological thrillers, recently had the opportunity to take my car to a racetrack, and have played bass guitar in a rock band.

I run Unscripted Therapy, a practice focused on helping people break out of stuck patters. I work with individuals who are tired of having the same conversations and somehow ending up right back where they started. Whether it’s anxiety, stress, or relationship patterns that keep repeating, something isn’t working—and at this point, that’s pretty clear. My approach is direct, collaborative, and focused on actually changing what’s not working. I’m not going to just sit there nodding while you vent and then ask how that made you feel for the tenth time. We’ll figure out what’s keeping you stuck and start doing something different about it. Therapy with me is honest, practical, and yes—sometimes a little blunt. But it’s also real. If you’re looking for something that actually helps you move forward (not just talk about why you’re stuck), we’ll probably be a good fit.

You may feel stuck in patterns that no longer serve you, unsure how to move forward, or simply exhausted from trying to hold everything together. You don’t have to navigate this alone. I’m a Licensed Clinical Social Worker (LCSW) with a certification in perinatal mental health (PMH-C). I specialize in supporting individuals through pregnancy, postpartum, and the early transition into parenthood. My experience working with families in the NICU is what really sparked my passion for this work. I saw firsthand how overwhelming, emotional, and isolating that experience can be—especially for parents of medically fragile or hospitalized newborns. That work continues to deeply shape how I support clients today. I work with people navigating a wide range of perinatal experiences, including anxiety, depression, trauma, identity shifts, and the emotional challenges that can come with becoming a parent. This period of life can bring a lot of unexpected feelings, and it’s not uncommon to feel unsure, anxious, or unlike yourself. I also recognize that opening up during pregnancy or postpartum can feel especially vulnerable, particularly when there’s fear of being judged or not feeling like the parent you “should” be. Many people carry a lot silently during this time, and it can take real courage to reach out for support. I aim to create a space that feels steady, supportive, and grounded, where you can talk openly about what you’re going through without feeling like you have to filter or hold back. If you’re navigating a difficult pregnancy, postpartum experience, or early parenting challenges, you don’t have to go through it alone.

Love and work are essential to a meaningful life. Relationships with others define experience and work gives a sense of purpose. Disturbance in these affect well being and contribute to an overwhelmed response leading to collapse. As trauma is relational so is recovery and healing. If you’re someone who carries a lot, who shows up for work, family, partnerships, motherhood, and community with strength and competence…but yet struggles with anxiety, self-doubt, boundary guilt, people-pleasing, overthinking, or a quiet sense of disconnection from yourself, please know that you are not alone. Many women appear composed while internally feeling exhausted or unseen. Often, the ways you cope in your roles (e.g. over-functioning, staying hyper-independent, avoiding conflict, or constantly striving) were shaped by childhood or traumatic experiences that once helped you survive. Since 2007, I’ve worked with women who want to understand how early attachment wounds and trauma histories continue to shape their current relationships, work, motherhood, and sense of self-worth. Therapy becomes a place to make sense of your patterns, and gently shift what no longer serves you. I work primarily with women, particularly women of color (BIPOC), and my approach is integrative, grounded in advanced training in: • EMDR • Internal Family Systems • Brainspotting (beginning March 2026) • Sensorimotor & Body-Oriented Psychotherapy • Havening Techniques • Yoga Therapy • Cognitive Behavioral Therapy (CBT) • Mindfulness-Based CBT This allows our work to include trauma processing, nervous system regulation, practical tools, relational exploration, and deeper meaning-making which including spirituality through a grounded, non-religious lens. The goal is integration: helping you feel more aligned, steady, and connected to yourself over time. If this resonates, I invite you to continue exploring my profile to learn more about my approach. Hello, I remember a joke from sometime ago that goes something like this: "Three friends were walking together one day, and one remarked, 'It's windy,' Another responded, 'No. It's Thursday.' Still the other answered, 'I'm thirsty, too. Let's have a cup of tea.'" Is it that way for you sometime? Finding that you are not really being heard and understood? I get it! I work with clients to help them find their strengths and abilities that assist them in better communicating their needs to themselves and to others which can lead to better outcomes in overall functioning in their lives. If that sounds like your experience, we can talk about it together.

*Currently accepting female clients only* Therapy has changed, and thankfully so. The old idea of spilling your heart out to someone who feels distant, judgmental, or out of touch does not work anymore. So let’s not do that. I’m Jessica Milgrome, a licensed mental health clinician with 15 years of experience, a Certified Rehabilitation Counselor, and a National Certified Counselor. I work with women navigating anxiety, overthinking, emotional overwhelm, relationship stress, burnout, and major life transitions. My approach is modern, direct, and judgment-free. Therapy with me is a real conversation. No awkward silence. No walking on eggshells. No wondering if you said the wrong thing. We talk openly, focus on what actually helps, and skip techniques that do not fit your real life. Sessions can be video or phone. If video feels too intense some days, that is okay. Therapy should feel supportive, practical, and human. To share a bit about myself, I’m a big fan of stand-up comedy because laughing really is life. I look forward to Mondays for Spotify’s Discover Weekly playlists and live by the mantra, “When words fail, let the music speak.” I also share my life with six dogs. I’m here to help you organize your thoughts, build confidence, set boundaries you can keep, and move forward feeling clearer and more grounded.

I am a Licensed Marriage and Family Therapist. I take a client-centered approach to treatment, ensuring that each session is tailored to the unique needs and goals of my clients. In my sessions, I would like to hear from you. Think of me as an observer in your art gallery, not as a critic, but as someone who appreciates your vision and the story you tell. You are the artist, and as such, you will explain to me your pieces, your vision, and your creations. I will take these all in and reflect back to you your work, and if appropriate, draw parallels from your earlier creations.
