I have worked in home health and hospice for most of my career with a strong emphasis on geriatrics and dementia care. For many years I have seen elders and their families in their homes but had to refer out any counseling other than crisis intervention to outpatient therapists. The problem is that I have been referring homebound people for outpatient therapy. This is not effective for many reasons.
