Grace A Acdan

PMHNP4 years of experience

As a board‑certified Psychiatric Mental Health Nurse Practitioner, (PMHNP) with passion for providing compassionate, evidence-based mental health care to individuals across the life span. I am dedicated to fostering strong, trusting therapeutic relationships and providing care rooted in respect, compassion, and professionalism. My goal is to create a safe, collaborative space where clients feel heard, supported, and empowered throughout their mental‑health journey. I approach each individual with genuine curiosity and empathy, honoring their autonomy while guiding them toward meaningful insight and sustainable growth. I bring over 18 years of experience in nursing and mental health, and I am deeply committed to providing care that honors the whole person. I am passionate about a holistic approach that considers not only mental‑health symptoms but also the social, environmental, and lifestyle factors that influence emotional well‑being.

I believe that meaningful progress happens when clients feel like true partners in their care, and I work intentionally to build a relationship grounded in openness, respect, and shared decision‑making.

My clinical focus includes the diagnosis and treatment of depression and anxiety, and I use a variety of evidence‑based modalities to support each individual’s unique needs. My treatment approach includes medication management, Cognitive Behavioral Therapy techniques, and psychoeducation to help clients understand their symptoms and build practical coping skills. I strive to create a collaborative, respectful, and empowering therapeutic environment where clients feel supported in their journey toward improved mental health and overall wellness.

First session

During your initial session, my priority is to get to know you in a supportive, comfortable, and non‑judgmental environment. I take time to actively listen so I can fully understand your concerns, experiences, and goals. This helps me gain a clear picture of what you’re navigating and what matters most to you. I present treatment options in a transparent and collaborative way, and together we create a plan that aligns with your needs, values, and preferences. I deeply respect your autonomy—you remain in control of your care, and I’m here to guide, support, and empower you. As we move forward, I regularly reevaluate your progress to ensure that our initial approach remains effective and responsive. If something isn’t working, we adjust it together. My goal is to help you feel supported, understood, and equipped with tools that genuinely help you move through life’s challenges. Present your symptomology of your present concerns, review your medical history, medication list, identify any allergies, collect family history, review tools such as: PHQ9, GAD, run a report on cures, ASQ (Ask suicide questions), sleep assessment, create a treatment plan which may include medication, discuss your goals for treatment-what you hope to gain from our work together and lastly, initial therapy, schedule 2 weeks follow up for medication management.

Ideal clients

As a nurse practitioner, I am passionate about supporting individuals who are navigating significant life transitions. I work closely with career professionals and new graduates entering the workforce, individuals experiencing the beginning or ending of relationships, and those balancing the emotional and practical challenges of caring for aging parents. I recognize that these periods of change can bring stress, uncertainty, and emotional strain, and I strive to create a supportive, non‑judgmental space where patients feel heard and empowered.

Treatment methods

Cognitive Behavioral (CBT)

In my practice, I incorporate Cognitive Behavioral Therapy principles during both initial assessments and follow‑up visits. I use these encounters to help patients understand the connection between their thoughts, emotions, and behaviors, especially when they are experiencing symptoms of anxiety, depression, or stress‑related conditions.
