*** note about visit pricing: Initial visit for medication management is $285, follow-up visits for established patients are $145. Hello! My name is Sarah Blizzard. I am a nurse practitioner who is board certified in both family practice and psychiatric mental health. I have been in practice for over 28 years as both a registered nurse and a nurse practitioner. I have extensive experience in psychiatric mental health and emergency medicine. I am skilled in treating a variety of mental health conditions such as anxiety, depression, substance abuse, OCD, Bipolar disorder, dementia, insomnia, traumatic brain injuries, ADHD, and much more. I work with patients ages 13 and older. I am in this field because I genuinely care about people and I want to help others to achieve their best quality of life possible. My goal is to help you reach your highest quality of life and become the best version of yourself! I am passionate about helping people gain access to mental health care and believe healthcare should be easily accessible, affordable, and of the highest quality. I am empathetic, compassionate, respectful, caring, and competent. I follow the model of taking care of patients as they were my own family. Healthcare can be scary and complicated to navigate, but it doesn't have to be, and I am passionate about helping you to make the process as easy and painless as possible.
Our first appointment will be a formal medical intake and evaluation. We will spend a great deal of time discussing your medical, surgical, and psychiatric history. We will also be collecting information about your family history, current symptoms, past medications tried, and more. The counseling provided in this appointment will include a discussion of treatment options, medication education (if appropriate), and supportive listening.
I do not offer strictly therapy. I am a Psychiatric Nurse Practitioner and I specialize in medication management and refer my patients to great counselors when needed as part of their overall treatment plan. I firmly believe that the patient-provider relationship is extremely important and involves trust, transparency, and teamwork. I consider myself a guide in your care, making suggestions based on scientific evidence and years of experience, but always letting the patient make their own decisions as to what is best for them.
Ages Served: 13 Years and Older I provide psychiatric care for patients ages 13 years and older and have extensive experience treating a wide range of mental health conditions. I have particular expertise in the treatment of anxiety, depression, obsessive-compulsive disorder (OCD), bipolar disorder, insomnia, and ADHD. My approach to care is individualized, evidence-based, and focused on providing safe, high-quality treatment that meets each patient’s unique needs. Please Note the Following Guidelines Before Scheduling an Appointment: Adolescent Patients (Ages 13-17) * I accept patients between the ages of 13 and 17; however, they must reside within approximately a 30-60 minute driving distance of my office in Winter Garden, Florida. * I do not provide care to patients under the age of 18 exclusively through telehealth. To ensure appropriate monitoring and continuity of care, treatment for adolescents utilizes a hybrid model that includes both in-person and telehealth appointments. ADHD and Stimulant Medication Management * I treat ADHD in both adolescents and adults and maintain comprehensive evaluation, monitoring, and treatment guidelines to ensure safe and appropriate care. * Patients who are currently prescribed stimulant medications or who may require stimulant treatment must reside within a reasonable driving distance of the office and be willing and able to attend in-person appointments as clinically indicated. * Certain medications, including some Schedule II stimulant medications, are not prescribed exclusively through telehealth and require periodic in-person visits and ongoing monitoring. *No guarantee is made that this practice will prescribe Schedule II stimulant medications. Treatment options and recommendations are made exclusively and at the discretion of the prescriber. If you have questions regarding my ADHD treatment policies, medication requirements, or whether my practice is the right fit for your needs, please contact the office prior to scheduling an appointment.

Sarah does a great job of really listening to me and taking my concerns seriously! She explains why she thinks the treatment she is prescribing is the best course of action, and she responds quickly when I have questions between sessions.

Smart, thorough, friendly person that focuses on what works for each patient. Easy to work with. Better than all the psychiatrists that I've seen the last 17 years. I highly recommend her.
