In the first session, my primary focus is on building rapport, conducting a comprehensive psychiatric evaluation, and creating a safe, nonjudgmental space for the patient. My approach is holistic—I assess not only psychiatric symptoms, but also medical history, social determinants, trauma history, substance use, and functional status. I begin by explaining the purpose of the session and the limits of confidentiality to set expectations. I use a mix of open-ended and structured questions to gather diagnostic information while also observing affect, thought process, and behavior. I conduct risk assessments as needed, especially for suicidality, self-harm, or psychosis. From there, I collaborate with the patient to identify key concerns and begin formulating a treatment plan. This might include discussing potential diagnoses, treatment options (e.g., medications, therapy, lifestyle changes), and setting short-term goals. I always aim to educate the patient and ensure they feel heard and understood, which builds trust for future sessions.
Clinical Judgment One of my greatest strengths is strong clinical judgment. I’m able to synthesize complex clinical information—including psychiatric symptoms, medical history, and psychosocial factors—to develop accurate diagnoses and individualized treatment plans. This helps me make thoughtful, evidence-based decisions, even in high-pressure situations. 2. Building Rapport and Trust I consider my ability to build rapport quickly with patients one of my key strengths. I create a safe, nonjudgmental space where patients feel comfortable sharing sensitive information.
