Hi there, I'm Ifeoma a board-certified Mental Health and Family Nurse Practitioner. I have a passion for meeting people where they are and upholding their dignity through a holistic approach to wellness. My mission is to provide a compassionate environment to help in the destigmatization of mental health and substance abuse disorders. Services offered include mental health evaluation and treatment, family medicine, office-based opioid treatment, and telehealth services. I offer an individualized treatment plan to meet various patient care needs. I promote the core values of compassion, respect, care, commitment, honesty, integrity, truthfulness, diversity in care and above all Christocentric in all aspects of care.
In the first session, the provider get to know the patient and the needs. Then, based on the identified needs, an individualized care plan will be structure to meet the patient care need, with on-going evaluation during follow up.
The Diagnostic and Statistical Manual of Mental Disorders DSM-5 and integration of evidence-based practice. Likewise, the use of other diagnostic tools and worksheets for the treatment of most mental health disorders like anxiety, depression, bipolar disorder, PTSD, schizoaffective disorder, Opioid use disorder, and ADHD.
