Heather Reichenbach
I am a board-certified Psychiatric and Mental Health Nurse Practitioner specializing in psychiatric evaluation and medication management. I bring extensive clinical experience in outpatient behavioral health, critical care, and geriatric psychiatry, along with over 20 years of nursing experience. My background in both acute and outpatient settings has strengthened my ability to assess complex clinical presentations, recognize overlapping symptoms, and develop thoughtful, individualized treatment plans. I work with adults experiencing anxiety, depression, trauma-related symptoms, and mood disorders, with a strong focus on diagnostic clarity and evidence-based care. I am especially experienced in geriatric psychiatry, where I provide compassionate care that addresses the unique mental health needs of older adults. I also work with adults across the lifespan and value a collaborative approach that emphasizes clear communication, trust, and patient involvement in treatment decisions.
First session
During your first session, we will complete a comprehensive evaluation. I will ask about your current symptoms, mental health history, past treatment, medications, medical history, family history, and relevant life experiences. We will also review the concerns that led you to seek care at this time and discuss what you are hoping to improve. Throughout the session, I encourage you to ask questions at any time so you feel informed and comfortable with the process. Open communication is an important part of our work together. We will also discuss your goals for treatment, whether that includes improving mood, reducing anxiety, managing stress, strengthening relationships, or feeling more balanced in daily life. At the end of the session, I will provide my initial clinical impressions and review potential treatment options. This may include medication management, therapy recommendations, or a combination of approaches based on your needs and goals.
Strengths
Experienced clinical judgment shaped by years of nursing care – With a background spanning critical care, outpatient behavioral health, and geriatric psychiatry, I am skilled at recognizing patterns, clarifying complex symptoms, and guiding treatment with precision. Thoughtful, individualized treatment decisions – I take time to understand the full context of your symptoms so that medication recommendations are purposeful, measured, and tailored to your needs. Clear communication and active collaboration – I make sure you understand your treatment options and what to expect, and I welcome questions at any point so decisions feel shared, not directed.
Ideal clients
I provide psychiatric evaluation and medication management for adults ages 18 and older. I have specialized experience working with both young adults and older adults age 65 and above. For young adults, I focus on supporting individuals through major life transitions such as college, career development, relationships, and increasing independence. These changes can often contribute to anxiety, depression, and difficulty adjusting. For older adults, I provide compassionate, individualized care with a strong interest in geriatric psychiatry. I understand the unique challenges that can arise later in life, including mood changes, cognitive concerns, grief, and changes in functioning and independence. In addition to these areas of focus, I work with a wide range of adults experiencing anxiety, depression, trauma-related symptoms, and mood disorders. Because mental health symptoms can be complex and overlapping, I take a careful and comprehensive approach to assessment and treatment. My practice is collaborative and patient-centered. I value clear communication, education, and shared decision-making. Alongside medication management, I incorporate attention to lifestyle factors, nutrition, and overall wellness to support long-term mental health and quality of life.
