Welcome. My name is Debbie. I am dual board certified as a Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner. I graduated from South University and Wilkes University respectively. I have been practicing for 5 years. I am bilingual in Spanish. I have 29 years of experience as a care giver to my special needs son. My approach is to meet your personal unique needs in a compassionate, empathetic, non-judgmental manner. During our sessions, my goal is to create a safe space where you feel heard, understood, and empowered to make the changes to reach your personal goals My focus is to do a thorough assessment, diagnosis, provide education, and properly treat your health condition with therapy and medication if needed. Together we will develop a personalized treatment plan. Medication management is used based on best evidence-based practice and utilizing Cognitive Behavioral Therapy (CBT). Although, I utilize my education and training, but also incorporate life experiences. I truly believe there is never a right or wrong time to start working on a more fulfilling and balanced life. You have taken the first step in your journey of healing by searching for a provider. If you’re ready to take the next step, I welcome you to reach out so that we can begin this journey together For the last couple of years, I have worked with a palliative program that allows me to service individuals with chronic medical conditions that are not ready for hospice services. I help to manage all their symptoms (depression, anxiety, insomnia, agitation, poor appetite, hallucinations, etc.), educate on end of life decisions (hospice services, DNR, advance care directives, etc.) and equip their care givers and families with techniques on how to provide the best care.

In our first session together, here's what you can expect

During our first session it would give us an opportunity to do personal introductions, complete a intake form of your past and current conditions, and we can establish individual goals and a plan of care. I will answer all of your questions. This will all be conducted in a safe and confidential video call. If you don't see a particular date/time- schedule an appointment and send me a message. I will try my best to accommodate.

The biggest strengths that I bring into our sessions

I am very transparent, compassionate, and focused on meeting your needs.

The clients I'm best positioned to serve

My dream clients are those struggling with depression, anxiety, burned out care givers, grief, geriatric clients with complex medical conditions such as dementia/alz,, those struggling with terminal medical conditions and end of life.

Dr. Diaz was articulate about how the mind and body work together, and her medical background shows in how she explains things. She made me feel heard and supported, especially with my depression, anxiety, and focus issues. She’s easy to talk to, empathetic. I felt empowered after my 1st session because I felt I’ve finally met the 1 professional who understands my issues. Ive searched for a doctor who truly gets it and has the education and experience to address my issues.
