(she/her)
If you’re struggling with mood swings, anxiety, difficulty focusing, or feeling like your current treatment isn’t working, you’re not alone. I’m TalBreshia Smiley, PMHNP-BC, and I specialize in helping adults find stability when things feel inconsistent, overwhelming, or unclear. I provide evidence-based care through thoughtful medication management, brief psychotherapy, and comprehensive ADHD evaluations. My approach is personalized and practical focused on helping you feel better in your day-to-day life, not just on paper.
My work centers on individuals navigating bipolar disorder, anxiety, ADHD, and perinatal or postpartum mood changes, especially those who feel stuck, sensitive to medications, or previously misunderstood in treatment. I take a collaborative, trauma-informed approach that helps you understand how your brain and body are functioning. Treatment is structured but flexible, with a focus on improving mood stability, focus, sleep, and emotional regulation, without unnecessary medications. I also incorporate CBT, mindfulness, and holistic strategies, considering lifestyle, environment, and biological factors (including GeneSight testing) to support long-term results. *Please Note* I do NOT prescribe (stimulants or benzodiazepines).
I show up as my genuine, grounded self in every session. I work with you, not just for you, creating a collaborative space where your voice guides treatment. My care is integrative and personalized. I consider lifestyle, nutrition, environment, and genetics (including GeneSight testing) to better understand your mental health and support long-term stability, not just symptom relief.
I work best with individuals who are ready for change, even if they feel overwhelmed or unsure where to start. My ideal clients are: Struggling with mood instability, anxiety, or focus issues Balancing work, parenting, or major life transitions Frustrated by past treatment or feeling overmedicated or unheard Open to a collaborative, structured approach If you’re looking for a provider who will listen, be honest, and help you build real stability, not just manage symptoms, we’re likely a good fit.
Cognitive Behavioral (CBT)
As your provider, my goal is to create a safe, nonjudgmental space where you feel heard, supported, and empowered. One of the therapeutic tools I often use is Cognitive Behavioral Therapy (CBT) because it helps us explore how your thoughts, feelings, and behaviors are connected.
Trauma Informed Care
I understand that many people carry experiences that have deeply shaped how they see themselves, others, and the world. That’s why I approach every session through a trauma informed lens with sensitivity, patience, and respect for your story and your pace.
Solution Focused Brief Treatment
I incorporate solution-focused therapy by helping clients identify their strengths, past successes, and practical steps toward meaningful change. Rather than dwelling solely on problems, I collaborate with clients to clarify goals, recognize what is already working, and build small, achievable actions that move them forward. This approach empowers clients, promotes hope, and supports efficient, goal-directed progress, especially well-suited for brief, focused therapeutic encounters.
Mindfulness-Based Therapy
I incorporate mindfulness-based therapy by guiding clients to develop greater awareness of their thoughts, emotions, and physical sensations in the present moment without judgment. Through grounding techniques, breathwork, and mindful reflection, I help clients improve emotional regulation, reduce reactivity, and increase resilience. This approach supports clients in responding more intentionally to stressors and complements brief psychotherapy by fostering skills that can be practiced both in and outside of sessions.

Warm, thoughtful, solution oriented, and cautious in a good way. Easy to talk to, comforting, and knowledgeable about various paths to take for one's mental health.
