Isabel Zapata
I am a board-certified Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) providing compassionate, evidence-based telehealth care to adults ages 21 through 64.
I take a holistic and patient-centered approach to care, focusing on both emotional well-being and overall functioning.
I offer psychiatric evaluations, diagnosis, and medication management for mild-to-moderate symptoms associated with ADHD, anxiety, depression, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), and clinically stable bipolar disorder. My approach is collaborative and personalized, with treatment plans tailored to each patient’s individual needs and goals. My practice provides scheduled outpatient care and does not offer crisis intervention, emergency psychiatric services, or after-hours/on-call coverage.
First session
During your first session, I will conduct a comprehensive psychiatric evaluation to better understand your current concerns, symptoms, medical and psychiatric history, previous treatment experiences, medications, lifestyle, and goals. By the end of the session, you will have a clearer understanding of my initial clinical impressions, recommendations, and next steps. If routine outpatient telehealth care is appropriate for your needs, we will develop a personalized treatment plan together. This may include medication, laboratory testing, review of additional records, or coordination with other healthcare providers when clinically indicated. Treatment recommendations are based on clinical need; a prescription is not guaranteed, and controlled medications are not initiated during the first session.
Strengths
I bring warmth, careful listening, and clinical thoroughness to each medication-management visit. I consider the full picture, including symptoms, medical history, previous treatment experiences, lifestyle, and goals, and explain my clinical impressions and medication options in clear, practical language. Rather than using a one-size-fits-all approach, I collaborate with each patient to develop an individualized treatment plan. I offer care in both English and Spanish.
Ideal clients
My ideal client is an adult between the ages of 21 and 64 experiencing mild-to-moderate, clinically stable mental health concerns and seeking greater clarity about their symptoms. I work best with individuals who value thoughtful medication management, open communication, shared decision-making, and a whole-person approach. They are willing to participate actively through regular follow-up and recommended monitoring as we work toward improved stability and daily functioning. My practice is designed for routine outpatient telehealth care and is not appropriate for psychiatric emergencies or conditions requiring a higher level of care.
Treatment methods
Supportive
I incorporate brief supportive psychotherapy into medication-management visits when clinically appropriate. Through active listening, validation, psychoeducation, encouragement, and focused problem-solving, I help patients better understand their symptoms, navigate current stressors, strengthen coping skills, and participate actively in treatment decisions. This supportive approach complements psychiatric evaluation and medication management but is not intended to replace ongoing or standalone psychotherapy when that level of care is needed.
