Hello. My name is Jack and I am a psychiatric/mental health and family health nurse practitioner. I received my masters in 2007 from the University of Central Florida, and my post-masters degree in 2020 from Regis College in Massachusetts. I have 5 years of psych/mental health experience working with clients suffering from common mental health conditions including, but not limited to: depression, anxiety, addictions, ADHD, OCD, PTSD, and bipolar disorder. Cash pay rates: Initial Psychiatric Evaluation (60 minute visit)--$125.00; Buprenorphine/Suboxone Evaluation (60-minute visit)--$125.00; 60-minute follow up visit with a Nurse Practitioner--$125.00; 45-minute follow up visit with a Nurse Practitioner--$90.00; 30-minute follow up visit with a Nurse Practitioner--$65.00; 20-minute follow up visit with a Nurse Practitioner--$50.00.
In an initial visit with me, following brief introductions, clients can expect to discuss the motivating reasons prompting the making of the appointment, and be educated on their treatment options which may or may not include medication prescription(s).
I try to take a collaborative approach to care by practicing the following principles: * providing education about available treatment options, * offering best-practice advice, * encouraging clients to take the lead on which treatment options would best suit their needs, * never being judgmental. As a psychiatric/mental health nurse practitioner, I have a personal mandate to do my best to establish a good working relationship and rapport with my patients. Clients who choose to work with me can expect to be supported and feel genuinely cared for at every turn of their journey towards better mental health.
I enjoy working closely and collaboratively with clients in an effort to improve their overall mental health by whatever means best meets their needs.
Motivational Interviewing
Through the utilization of open-ended questions, reflective listening, and summarization statements, I try to gain a better understanding of what the client is experiencing, and how I can best assist them on their path to improved mental health.
Psychoeducation
I utilize psychoeducation by providing accurate and understandable information about a specific mental health diagnosis, including its causes, symptoms, progression, prognosis, and available treatments (including medication and therapy options).

I can’t recommend this provider enough. he is extremely thorough and takes the time to really listen in order to understand what I’m experiencing, rather than rushing through appointments. His approach is very solution-oriented, and he provides clear guidance that actually helps. He is highly knowledgeable about medications and treatment options, which makes me feel confident in the care I’m receiving. On top of that, he is easy to talk to and create a comfortable, supportive environment.
