Hi, I’m Melanie Graham, MSN, APRN, PMHNP-BC, a board-certified Psychiatric Mental Health Nurse Practitioner. I believe meaningful change doesn’t always require having the entire path figured out. Sometimes, it simply begins by recognizing that where you are isn’t where you want to stay—and deciding it’s time to pivot. I work with adults experiencing anxiety, depression, ADHD, mood disorders, trauma-related concerns, and other mental health challenges. My approach is compassionate, collaborative, and individualized. I’ll take the time to understand your symptoms, experiences, concerns, and goals—not just your diagnosis. Through psychiatric evaluation, medication management, education, and supportive conversation, we’ll work together to develop a treatment plan that feels realistic and appropriate for you. I want you to understand your options, feel comfortable asking questions, and have an active role in your care. At Pivot Pointe Psychiatry, change doesn’t have to happen all at once. Sometimes, one thoughtful pivot can be the beginning of a new direction.

First session

Your first session is an opportunity for me to understand you as a whole person—not simply a diagnosis or a list of symptoms. We’ll talk about what brought you to treatment, what you’ve already tried, what is or isn’t working, and what you hope to change. My patients often tell me that I’m a good listener and willing to go the distance to understand what may be helpful for them. I don’t believe in giving up simply because the first approach doesn’t work. We’ll explore your options together, and I’m willing to ask questions, dig deeper, and continue learning when your individual needs call for it. You can expect an individualized treatment plan, clear explanations of my recommendations, and an opportunity to ask questions and participate in decisions about your care. My goal is for you to leave your first session feeling heard, understood, and with a clear direction for what comes next.

Strengths

I bring a person-centered, compassionate, and collaborative approach to each session. I believe you are the expert on your own experiences, and my role is to listen, ask thoughtful questions, provide clinical guidance, and work with you to develop a treatment plan that reflects your individual needs, preferences, and goals. As a Psychiatric Mental Health Nurse Practitioner, I bring strong skills in psychiatric assessment, diagnosis, medication management, risk assessment, psychoeducation, and evidence-based treatment. My nursing background has also taught me to look at the whole person, recognizing that physical health, mental health, relationships, life experiences, stressors, and environment can all influence how someone feels and functions. I value open communication and shared decision-making. I want my patients to understand their diagnoses, medications, treatment options, and the reasoning behind my recommendations. I strive to create a comfortable, nonjudgmental environment where patients feel heard, respected, and able to be honest—even when things are difficult. Above all, I bring empathy, clinical experience, curiosity, and practicality to my sessions. Treatment should not simply look good on paper; it needs to make sense for the person living it.

Ideal clients

I work well with clients who recognize that something in their current situation, thought patterns, or approach to their mental health is no longer working and are ready to pivot toward something different. You may be struggling with anxiety, depression, ADHD, mood changes, past trauma, or simply feeling stuck and unsure of what needs to change. You do not need to have everything figured out before coming to treatment. What matters is a willingness to be open about your concerns, explore your current mindset, and participate in your care. Together, we can identify your goals, consider treatment options, and work toward meaningful changes that help you feel and function more like yourself.

Treatment methods

Cognitive Behavioral (CBT)

CBT (Cognitive Behavioral Therapy) I use CBT techniques to help clients recognize connections between thoughts, emotions, and behaviors. Together, we identify patterns that may be keeping you stuck and practice reframing, problem-solving, coping skills, and behavioral changes that support healthier responses. DBT (Dialectical Behavior Therapy) I incorporate DBT-informed strategies to help clients strengthen emotional regulation, distress tolerance, mindfulness, and interpersonal effectiveness. These skills can be especially helpful when emotions feel overwhelming or reactions interfere with relationships, goals, or daily functioning. Motivational Interviewing I use motivational interviewing to create a nonjudgmental space to explore uncertainty about change. Rather than telling you what you “should” do, we’ll identify what matters to you, work through barriers and ambivalence, and strengthen your own motivation and confidence to move forward. Behavioral Activation When depression, low motivation, or avoidance makes it difficult to engage in life, I use behavioral activation to help clients take manageable steps toward meaningful activities and routines. We focus on realistic goals that can gradually improve functioning, confidence, and mood—because sometimes the first pivot toward feeling different is doing something new.
