My name is Tajae Sergeon, the heart behind Woven With Grace Therapy Practice — a space created for real people navigating real emotions, real struggles, and real growth. I believe therapy isn’t about perfection; it’s about finding wholeness in the threads of who you already are. Healing can be tender, raw, and even a little playful — and that’s okay. Here, we embrace the messy parts of being human with compassion, curiosity, and a deep respect for your story.
Your first session is a gentle introduction — a chance for us to get to know each other and begin building a comfortable, trusting space for your healing. We’ll start by talking about what brought you in and what you’re hoping to gain from therapy. You don’t need to have everything figured out; sharing whatever feels comfortable is enough. I’ll ask some thoughtful questions to better understand your experiences, goals, and any challenges you’re facing. We’ll also talk through things like: what therapy looks like and how we’ll work together, my approach and the evidence-based tools we may use, what support feels most helpful for you, any questions or concerns you might have, The pace is always your own. There’s no pressure to dive into anything before you’re ready. The first session is about making sure the space feels safe, supportive, and a good fit for both of us. My goal is for you to leave that first meeting feeling understood, respected, and confident that you have someone walking beside you as you begin this process.
Grace is at the heart of everything I do. It shapes the way I listen, the way I respond, and the space I create for you. From grace’s perspective, therapy is not about perfection, pressure, or proving anything — it’s about being welcomed exactly as you are. Through this lens, therapy becomes a place where mistakes are treated with understanding, setbacks are met with patience, and your journey is honored rather than rushed. Grace invites curiosity instead of criticism, gentleness instead of shame, and support instead of pressure. My role is to reflect that grace back to you — through evidence-based care, thoughtful presence, and a commitment to seeing your humanity above all else. Grace guides the process, making room for real healing, real connection, and real transformation.
I work best with individuals and couples who are motivated for growth, even if they are unsure where to begin. I serve adults and young adults across diverse gender identities and backgrounds who are navigating anxiety, depression, life transitions, relationship challenges, and emotional overwhelm. I am particularly well-suited for clients who are seeking deeper self-awareness, improved communication, and healthier coping strategies. My ideal clients are open to reflection and willing to engage in the therapeutic process, whether that means building practical skills, processing past experiences, or exploring patterns that impact their daily lives. I also work well with couples who are committed to strengthening their relationship, rebuilding trust, and developing more effective ways to connect and resolve conflict. I strive to create a supportive, nonjudgmental space where clients feel safe to be authentic while also being gently challenged toward growth, accountability, and meaningful change.

Psychodynamic
My psychodynamic approach emphasizes exploring unconscious patterns, early life experiences, and relational dynamics that influence current functioning.
